目的:分析静脉用药调配中心的药品损耗原因,优化管理措施,减少药品损耗的发生。方法:对调配中心发生药品损耗的原因进行分析总结,然后采取相应的整改措施,比较2014年7-12月和采取改进措施后2015年1-6月差错事件发生情况。结果:未实施改进前2014年7-12月总调配输液数为281 381袋,其中发生失误425次,占0.15%,平均每月失误70.8次;改进后的2015年1-6月总调配数为301 095袋,发生失误157次,占0.05%,平均每月失误26.2次。导致药品损耗的内部因素(排药、核对)中破损的失误率下降了63.1%,成品冲错的失误率下降了61.5%,漏液失误率下降了76.5%。结论:多环节的干预和优化措施有效的减少了药品的损耗,提升了药学服务的质量。
OBJECTIVE: To analyze the cause of drug loss in intravenous drug deployment center, optimize management measures and reduce the occurrence of drug loss. Methods: To analyze and summarize the causes of drug loss in the allocation center, and then take appropriate corrective measures to compare the occurrence of error events in January-June 2015 and July-December 2014 and improvement measures. Results: Before the implementation of the improvement, the total number of infusions administered during the period from July to December 2014 was 281 381 bags, of which 425 were mistakes, accounting for 0.15%, with an average of 70.8 mistakes per month. The revised total number of January-June 2015 deployment 301,095 bags, 157 errors occurred, accounting for 0.05%, with an average of 26.2 mistakes per month. The error rate of damage in the internal factors (drug discharge, check) that caused the drug loss decreased by 63.1%, the error rate of finished product error decreased by 61.5% and the error rate of liquid leakage decreased by 76.5%. Conclusion: Interventions and optimization measures at multiple levels effectively reduce drug consumption and improve the quality of pharmacy services.
